ENROLLMENT:

WAITING LIST: To be considered for future kindergarten enrollment or if you would like to be placed on a waiting list for a class that is presently full, simply complete the Waiting List Request form, found in the back of this folder, and send it to Crestview Academy at the address above.  Open positions are filled on a first-come, first-served basis.  A fee is not needed if you are submitting a Waiting List Request form  Waiting List/Inquiry Request form.

HOW TO ENROLL: If you are interested in submitting an application for your child to attend Crestview, please contact us to ensure that there is an opening for enrollment in that grade.  We will then send you an application form to complete and return to Crestview Academy, 1519 Peterson Avenue, Eau Claire, WI  54703, along with a $50 nonrefundable application-processing fee.  Upon receipt of your application we will set up a meeting with you for screening/placement assessment.  Crestview Academy does not discriminate and admits students of any gender, religion, race, color, and national or ethnic origin.  Application does not guarantee acceptance.
